California AB 506 – Background Check/Livescan Policy for CA Volunteers

California AB 506 (Effective January 1, 2022)

Effective, January 1, 2022, all administrators, employees and regular volunteers in California MUST have a one-time background check as approved by the California Department of Justice to exclude individuals with a history of child abuse. This form of background check requires live scan fingerprinting.

To clarify, a REGULAR VOLUNTEER is a person who has direct contact with children 16 hours a month or 32 hours per year, which applies to most coaches, referees, team parents, and board members who spend 2 or more hours on the field weekly.

To help streamline the process, we have partnered with ApplicantServices.com. Through this process, the cost has been reduced to $20 – $25 per volunteer based on the process you use. If you are fingerprinted at a live AYSO event the cost is $20 and $25 at a Live Scan Service Center (UPS Stores). Please understand that this specific process has been negotiated specifically for AYSO, so you will need to register via the link provided below to receive this cost discount and to ensure the information is sent to AYSO directly from the California Department of Justice.

Please note that Livescans are not transferrable from another organization, so you need to perform this process for each applicable organization you volunteer with, but the good news is that this is a one-time requirement.

Dear Volunteers,

Confidential Live Scan will be conducting Live Scans on the above date for all Regions in Area 11L. All volunteers needing to get their Live Scan completed are invited to get fingerprinted. Frank Bernal, Region 84 referee and Live Scan Technician/Owner of Confidential Live Scan, will be conducting the fingerprinting. Confidential Live Scan is a CalDOJ certified Live Scan fingerprinting provider, authorized to provide fingerprinting services for AYSO.

On the day of the event, you will need to present your unexpired California Driver's License or unexpired Other photo ID (passport, military ID, alien registration card, California ID Card, or out of state driver’s license), no exceptions please. Upon arrival, please check-in with Frank. Each scan only should take a few minutes.

Please complete, print and bring the attached Request for Live Scan Service Form to the Live Scan event per the below instructions. Please read the attached Privacy Disclosures. The disclosures are for your information only and need not be printed.

INSTRUCTIONS FOR COMPLETING REQUEST FOR LIVE SCAN SERVICE FORM

Please complete one copy of the attached fillable PDF Form, print the Form and present the Form to Frank when it is your turn to get fingerprinted. In order to use the fillable form, you need to download the form and/or have Adobe Acrobat installed. In lieu of completing the fillable fields on your electronic device, you may print the form and legibly hand write the required information. Complete only the highlighted fields as follows:

Last Name and First Name Fields: Enter your name as shown in your photo identification listed in item 5 or 6 below. Do not enter shortened names or nicknames for your first name.
Middle Name Field: Enter your full middle name. If you don’t have a middle name, leave it blank.
Suffix Field: Enter Jr., Sr. or Roman Numeral(s) only. If not applicable, leave blank.
Date of Birth Field: Enter, month, day and year
Driver's License Field: Only enter a valid unexpired California Driver's License (CADL). Do not enter a California Identification Card or out of state/country driver’s license in this field. If you do not have a valid unexpired CADL, other forms of identification may be entered as described in item 6 below. A valid unexpired CADL, while not required, is preferred above other forms of identification.
In the Misc. Number (Other Identification Number) Field: Complete ONLY if you do not have a valid unexpired CADL. Other forms of acceptable (photo) identification include an unexpired passport, California ID Card (not a CA driver's license), military ID or out of state driver's license. Please include the number and type of Other Identification Number ( i.e., Nevada DL 12345, U.S. Passport 6789). If you do not have one of the above identifications, please contact Frank for other forms of acceptable identification.
Home Address Field: Enter your full address, including apartment number, if applicable, and Zip Code.
Your AYSO Region Number Field: Enter your AYSO Region Number, 889 (Aliso Viejo), 84 (Mission Viejo), 85 (Lake Forest), 1455 (Ladera Ranch), 630 (RSM), 1678 (RMV), 41 (Laguna Niguel).
After printing the form, sign and date in the appropriate fields.
